Responsibilities
* Support the Practice Pharmacist in delivering medicines optimisation services Undertake structured medication reviews for patients.
* Assist with repeat prescription requests, including medication queries and synchronisation.
* Respond to medication-related queries.
* Support the management of long‑term conditions through medication monitoring.
* Assist in audits and quality improvement initiatives.
* Promote safe, cost‑effective prescribing in line with local/national guidance.
* Support the delivery of QAIF/QOF targets.
* Assist in achieving prescribing and medicines indicators.
* Searches and reports, including high‑risk medications and monitoring requirements.
* Contribute to practice performance improvement through audit cycles analysis.
* Work collaboratively with the wider team to ensure compliance with local and national quality standards.
* Support and conduct acute medication clinics under supervision.
* Assess and manage minor illness presentations within scope of practice.
* Provide timely prescribing advice and treatment recommendations.
* Escalate complex cases to the GP or Practice Pharmacist as required.
* Promote safe prescribing and patient understanding during acute consultations.
* Process prescription requests and ensure accuracy of medication records.
* Update and maintain patient medication information on clinical systems.
* Support coding and documentation of medication‑related activity.
* Assist with practice‑level prescribing reports and data collection.
* Contribute to medicines safety alerts and recalls.
