Job Summary
We are looking for an enthusiastic and experienced nurse to join our exciting service in Furness - The Integrated Wellness Service. We are a multidisciplinary team providing dedicated support for those patients who are at high-risk of admission to hospital and those who are recurrently being admitted.
The programme aims to beCo-ordinated,Holistic,Intensive,Early andFocussed, bringing together colleagues from medicine, nursing, pharmacy, social care, voluntary sector, therapy and community teams.
You will undertake full assessments of health and social needs, identify those at high risk of admission, and intervene early to reduce that risk.
As Case Manager, you will promote good practice and influence service development to improve patient care. The successful applicant will need to develop strong relationships with the teams involved in the MDT and wider system partners and work closely with the Clinical Lead and Team Lead. This is an opportunity to work within an exciting new model of care in Furness.
Main duties of the job
We are looking for a dynamic and skilled Case Manager to join our Integrated Wellness Service team. This is an exciting opportunity to work in a supportive environment where your expertise and initiative will drive meaningful patient outcomes.
Our data identified a very small number of patients (<1000) who drove ~30% of our admissions and bed days at Furness General Hospital. With your input, we want to see the Integrated Wellness Service address the diverse, unmet needs in these patients and then support them to 'live well' in their home environment. As this role is a community role you will be required to travel to visit patients in their own homes.
As a Case Manager, You Will Provide Holistic, Evidence-based Care To Individuals At High Risk Of Hospital Admission Or Those Frequently Admitted. You Will Manage a Complex Caseload, Collaborating With Multidisciplinary Teams To Develop, Implement, And Evaluate Personalised Care Plans. Key Responsibilities Include:
Supporting admission avoidance and ensuring high-quality patient care through reactive case working.
Leading holistic care planning and providing specialist advice to patients and carers.
Acting as a role model and mentor to junior staff.
Monitoring and improving patient outcomes, including admission and readmission rates.
Developing clinical expertise and contributing to service improvement.

About Us
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including GP Practices, Millom Hospital and GP Practice, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
