Patient Pathway Coordinator

We are looking for a motivated and enthusiastic individual to join the Trauma & Orthopaedics service as a patient pathway coordinator focusing on 18‑Week RTT pathways.
In this role you will be responsible for the validation, investigations and processing actions for patient pathways against the last clinic letter and clinical systems to ensure patients are accurately recorded on the Trust Patient Administration System (PAS) and RTT timescales are met. The purpose of this role is to improve RTT pathways and ensure patients receive timely treatment.
Responsibilities
* Ensure every patient pathway is tracked to be diagnosed and treated as quickly as possible, achieving relevant RTT standards.
* Maintain an up‑to‑date record of patient tracking and escalations using the Trust's database FDP.
* Escalate patients' diagnostics and treatment appointments as needed.
* Book outpatient appointments and bring forward when required.
* Support the Team Leader and Service Manager to ensure administrative processes are delivered to a high standard.
Qualifications
* Working knowledge of Cerner EPR system.
* Previous experience in the Trauma & Orthopaedics service.
* Previous experience working as a patient pathway coordinator (PPC).
* Working knowledge of FDP.
* Working knowledge of RTT pathways.
