We are seeking to appoint an enthusiastic, motivated and reliable Community Therapy Rehab Assistant to work with our the therapy team within Rural Alliance Care Community Team based at Chowley Oak. The successful applicant will undertake follow up visits and undertake prescribed therapy exercise programmes on behalf of the therapists within the team.
You will be working as part of a multi-disciplinary team providing care to patients in their own homes, driving is an essential part of this role.
The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this.
To work alongside the therapists within team to support patients.
Undertaking review of equipment.
Exercise programmes with patients in their own homes.
Supporting on joint visits with therapists.
