The post holder will work as a member of the Continuing Healthcare registered nurse team to assess, plan, implement and evaluate care under the direction of the Case manager. To monitor competency levels of healthcare support workers to ensure the provision of effective, efficient, appropriate quality care to patients with complex needs in the community.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

An exciting opportunity has arisen for Registered Nurses to join our innovative and developing community nursing team. The Care At Home Team provide high quality, safe and bespoke care to individuals within their own homes.
As a Registered Nurse within the Care At Home team you will experience variety within your role. The opportunity available will be split between delivering bespoke holistic care within our community packages of care and working from our community bases. This will enable you to develop both your clinical and leadership skills.
Applications are welcomed from dedicated individuals with a passion for delivering high quality nursing care to individuals in the community. The role is challenging but rewarding and you will be fully encouraged and supported with your professional development. The Care At Home Team has a dynamic internal education team with a focus on nurse development which aids succession planning.
Full UK Driving Licence is essential for this role.
