Overview
The post holder will be the point of contact for all administrative issues relating to patient\'s referral and appointment bookings. The post holder will work as part of a multi-disciplinary team and will provide support to Senior Sister or Medical Day Unit Nurse-In-Charge in proactive management and screening of incoming referrals, booking appointments, sourcing clinical documentation and diagnostic results from internal and external care providers, communicating with members of multi-disciplinary team and patients and ensuring pathways are managed smoothly.
Responsibilities
* Be the point of contact for administrative issues relating to patient referrals and appointment bookings.
* Provide support to Senior Sister or Medical Day Unit Nurse-In-Charge in proactive management and screening of incoming referrals.
* Book appointments, source clinical documentation and diagnostic results from internal and external care providers.
* Communicate with members of the multi-disciplinary team and with patients to ensure pathways are managed smoothly.
* Ensure high levels of patient and referrer satisfaction by being accessible, customer focused and a knowledgeable point of contact.
* Provide a fast, efficient and effective service in accordance with trust and ward policy.
* Liaise directly with patients and the referring team.
* Demonstrate good IT skills and an excellent telephone manner.
* Communicate professionally with a range of professionals within the Medical Day Unit team.
