Job overview

Do you have a friendly 'can do' approach to life, and experience working as part of a health or social care team ?

We are looking for you to work as as part of the Amersham Rapid Response & Intermediate Care team supporting patients in their own homes, providing personal, social and rehabilitative care. The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate admission of patients into Hospital and facilitate timely discharge into the community.

The team is committed to ongoing education on the job and development opportunities are available within the organisation. Successful applicants If who don't already have a Level 3 Qualification in Health and Social Care will be expected to work towards that in post.

Main duties of the job

Your main duties include:

1. Providing general and specific care to adults within competencies
2. Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, providing defined delivery of nursing, social and rehabilitative care to patients in liaison with senior staff.
3. Taking responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients’ own homes.
4. Being responsible for the completion of delegated non-clinical tasks within the team.

Your main responsibility is to :

5. Improve the care of patients in the community, so patients can remain safely in their own home for as long as possible
6. Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery
7. Willingness to further develop your clinical expertise in a range of areas
A full UK driving licence and access to a car with Business insurance for work purposes

Experience

Essential criteria

17. Previous NHS/Social Care/ Voluntary Organisation work experience.

Desirable criteria

18. Previous caring experience of working in another Health or Social Care environment.

Education, Qualifications & Training

Essential criteria

19. Good literary and numeracy skills.
20. Health and Social Care Level 2 or 3 Diploma or equivalent relevant patient facing experience and willingness to undertake Level 3 qualification.
21. Have the ability to undertake training to acquire the skills appropriate for development in the post.

30. Previous NHS/Social Care/ Voluntary Organisation experience.

35. Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire.

PPE requirements: Buckinghamshire Healthcare NHS Trust requires all colleagues to wear appropriate personal protective equipment (PPE) in accordance with our infection prevention and control procedures.

COVID-19 and Flu vaccinations remain the best way to protect yourself, your family, your colleagues and of course our patients from the viruses when working in our healthcare settings. We encourage our staff to be vaccinated when recommended.
