Integrated Care Clinician (Paramedic) - Llangunnor
The post holder will undertake telephone triage assessments of patients efficiently and effectively.
They will implement or advise on the most appropriate intervention to meet patient needs, following Trust policies, protocols, and guidelines.
Work primarily in an autonomous capacity, assessing, advising, and treating callers or clients within established guidelines.
Proficiency in Welsh is desirable; applications from English and Welsh speakers are welcome.
Responsibilities include conducting safe and effective telephone clinical assessments, providing advice, and, if necessary, referring patients to the appropriate agency or professional for a wide range of conditions.
Exercise professional autonomy and clinical judgment by critically analyzing complex information using:
* Clinical Decision Support Software (CDSS)
* A symptomatic assessment framework
* Emergency Communication Nurse System (ECNS)
Possess strong clinical skills and sound judgment supported by experience and knowledge.
Refer to multidisciplinary team members as appropriate.
Manage calls safely, appropriately, and evidence-based according to the caller’s needs.
Identify emergency situations and redirect care accordingly.
Provide self-care advice to empower callers to manage symptoms at home when suitable.
Support callers to make informed choices by advising on the correct use of primary care, emergency services, and other health or social care organizations.
