Remote – Home Based (travel expenses covered)

The Role:
As a Rehabilitation Case Manager, you’ll:
•Manage your own caseload and diary remotely
•Conduct Initial Needs Assessments & home visits (travel covered)
•Design and coordinate tailored rehabilitation programmes
•Collaborate closely with our expert multidisciplinary team
•Communicate with patients, healthcare providers, insurers & solicitors
•Produce high-quality clinical reports and care plans
About You:
* Qualified Occupational Therapist, Physiotherapist or Nurse
* 3+ years post-graduate clinical experience
* HCPC, NMC or GMC registration
* Current knowledge in complex ortho, neuro, MSK, or psychological injuries (brain injury experience desirable)
* Strong clinical reasoning, report writing, and communication skills
* Confident working independently and collaboratively within an MDT
* Own vehicle and willingness to travel (up to 3 hours)
* UK-based candidates only (we do not offer sponsorship)
Private case management experience not required — we provide full training & mentoring.
