Job description
We are seeking a compassionate and dedicated Social Worker to join the Adult Community Team. As a vital member of the team, you will be playing a key role in providing essential support and services to adults in the community, empowering them to lead fulfilling lives and promoting their overall well-being.
Responsibilities include, but are not limited to:
* Conducting comprehensive assessments of individuals' social care needs, considering physical, emotional, and environmental factors.
* Developing and implementing tailored care plans in collaboration with service users, promoting independence and resilience.
* Providing crisis intervention and support during challenging situations, working closely with other professionals and agencies.
* Working collaboratively with healthcare professionals, local authorities, and community organisations to provide holistic care.
* Attending multi-disciplinary meetings, contributing valuable insights and expertise to enhance the overall care and support network.
* Fostering positive relationships with external agencies to strengthen the support available to service users.
* Maintaining accurate and up-to-date records of assessments, interventions, and service user progress.
* Preparing reports for case conferences, reviews, and legal proceedings as required, ensuring compliance with data protection and confidentiality policies.
Requirements:
* Qualified Social Worker with a recognised Social Work degree.
* Previous experience in adult social care and community-based settings.
* Strong knowledge of relevant legislation, policies, and procedures.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a multidisciplinary team.
* Commitment to maintaining professional boundaries and promoting ethical practice.
