Job summary
We are a team of Occupational Therapists and Physiotherapists working closely with the multidisciplinary team to prevent further deterioration in patients' physical functioning and wellbeing. We cover the areas of Chorley, Preston and South Ribble.
Your role will be to support the Occupational Therapist and Physiotherapists within the team at Longridge Community Hospital including undertaking non-complex assessments and carrying out treatment plans under the direction of the therapists.
Responsibilities
The role involves working with patients in their own homes and a variety of community settings including Longridge Hospital. You will require the ability to travel to carry out home visits.
You will undertake non-complex assessments and carry out treatment plans under the direction of the therapists, supporting the Occupational Therapy and Physiotherapy interventions within rehabilitation and admission avoidance.
Qualifications
* Assistant Practitioner Qualification or equivalent
* NVQ level 3 or equivalent
* Evidence of continued professional development
Knowledge
* Knowledge of long term conditions
* Knowledge of mental illness and disability
* Knowledge of the cultures and religions of the area communities and the ability to work across those cultures
Experience
* Experience of rehabilitation in the community
* Demonstrates the ability to work with minimum supervision
* Is able to organise own time and workload
