We are a dedicated and leading health and social care provider supporting individuals with Learning Disabilities, Autism, and other complex needs to live independently in their own homes and communities. Our Outreach Service focuses on delivering person-centred, outcome-based support, empowering people to achieve their goals and enhance their quality of life.

The Role

As aTeam leader/Senior Support Worker, you will play a key role in leading by example, supporting the management team, and ensuring the highest standards of care and support are provided. You will work alongside Support Workers, guiding and mentoring them to deliver personalised support plans, while also undertaking hands-on care when required.

Duties and Responsibilities:

* Develop and guide care staff in creating and implementing personalised Keywork session plans that promote customers' independence, safety, budgeting skills, physical and emotional wellbeing, and emotional regulation.
* Work proactively and holistically with customers to ensure, wherever possible, that their physical, emotional, psychological, educational, recreational, and social needs are met.
* Understand and support the rota system to ensure adequate staffing levels and effective deployment of staff.
* Provide guidance and support to care staff, ensuring they understand their roles and responsibilities.
* Develop and implement personalised support plans, Risk assessment, PBS, andworking collaboratively with the multidisciplinary team.
* Encourage and support customers to develop routines, gain independent living skills, including cooking, shopping, laundry, and budgeting, in preparation for independence.
* Prompt or support customers to access the local community as required, attend appointments, support contact, access education, employment, or training.
* Signpost customers to appropriate external services.
* Report safeguarding concerns immediately to management or the on-call manager if out of hours.
* Engage customers in meaningful activities and facilitate engagement by prompting or accompanying them to outdoor activities as required.
* Respond to and manage situations that challenge calmly and effectively.
* Work as part of a supportive team and carry out duties in a professional manner, refraining from gossiping and discussing other customers not residing at the placement.
* Participate in team meetings and supervision as required.
* Prepare information and represent the company/advocate on behalf of customers/support them to have their views heard in professionals' meetings and reviews
* Undertake additional ad hoc duties as required to support the care team and ensure the smooth running of the service.

Requirements:

* Have a minimum of 2 years' experience working in a similar environment and/or be qualified to NVQ Level 2 Health and Social Care, or hold a professional qualification such as teaching, social work, or therapy.
* Strong leadership and communication skills.
* Ability to work flexibly and prioritize tasks effectively.
* Knowledge of care planning, risk assessment, and person-centred support.
* Proficient in Microsoft Office, particularly Excel, for rota management, reporting.
* Ability to manage behaviours that challenge and provide emotional support to customers.
* Commitment to providing high-quality care and support to customers.
* Ability to think critically and respond effectively in unexpected situations, with strong problem-solving skills.

Desirable Qualifications:

* Supervisor, team leader or senior care worker experience.
* Experience with rota management or similar systems.
* Knowledge of relevant legislation and regulations (e.g., Care Act, Mental Capacity Act).
