Job Summary

Clinical Pharmacist role at PartnersHealth. This position is based at Keyworth Health Centre, Keyworth, Nottingham NG12 5JU, and supports multi-site delivery within the practice and PCN pharmacy team. The pharmacist will provide clinical medicines management across a range of duties, including day-to-day medication queries, acute prescription requests, hospital discharge prescribing and follow-up, MHRA alerts, and targeted medication reviews. The role includes developing safer prescribing protocols to meet CQC expectations and acting as a specialist resource for medicines-related queries.


Main duties and responsibilities

* Work within professional boundaries as part of a multi-disciplinary team in general practice.
* Take responsibility for chronic disease management areas within the practice and conduct clinical medication reviews for patients with complex polypharmacy.
* Provide primary support to practice staff on prescription and medication queries, support repeat prescribing, handle acute prescription requests, and manage medicines reconciliation on transfer of care; implement safer prescribing systems.
* Offer clinical leadership on medicines optimisation and contribute to quality improvement, and manage aspects of the quality and outcomes framework and enhanced services.
* Ensure integration with community and hospital pharmacy to optimise skill mix, improve patient outcomes, and manage workload.
* Demonstrate commitment to improved quality of care and operational efficiency within general practice.
* Deliver medicines optimisation through holistic medication reviews and support safe discharge from hospital or clinic letters.
* Provide telephone, domiciliary and face-to-face medication reviews; support long-term condition management as appropriate.
* Participate in education and training for practice staff and clinical placements; contribute to local research and audit activities.


Areas of responsibility for all clinical pharmacists


Clinical Practice

* Provide medication reviews and long-term condition management; prioritise polypharmacy, frailty, asthma, COPD, and care for people with learning disabilities.
* Assist with medicines reconciliation of hospital discharges; support self-care and reduce waste through structured reviews.
* Critical appraisal of medicines information and communication with patients, carers, and health professionals.
* Maintain documentation in patient records and communicate with other health professionals accordingly.


Education and Training

* Provide training and teaching sessions to practice healthcare professionals and admin staff; engage in CPD with senior pharmacists.


Leadership and Management

* Support the medicines optimisation agenda across the PCN; ensure safe prescribing and antimicrobial stewardship; contribute to Network Contract DES and enhanced services; pursue ongoing professional development.


Clinical Governance and Quality

* Lead clinical audits within the PCN; report significant learning events; manage adverse drug reactions communications with MHRA; respond to drug recalls/alerts.

* Promote equality and diversity; ensure services meet needs of all communities; ensure CQC compliance where medicines are involved.

* Be professionally autonomous, responsible for own work, comply with professional standards and data protection requirements; work unsupervised within professional boundaries.

* Travel between practice sites; occasional contact with body fluids; comply with health and safety policies and reporting systems.


Person Specification

Essential:

* Mandatory registration with the General Pharmaceutical Council
* Significant post-qualifying experience as a pharmacist
* Commitment to reducing health inequalities; good communication skills; ability to work under pressure; leadership and teamwork capabilities
* DBS clearance and current UK registration

Desirable:

* Membership of Primary Care Pharmacy Association; Royal Pharmaceutical Society; independent prescribing qualification; GP clinical systems experience


Qualifications & Training

* Undergraduate degree in pharmacy; understanding of standards of practice; performance management policies
* Desirable: independent prescribing qualification; clinical diploma or significant clinical experience
