Overview
This is a Band 6 Community Learning Disability Nurse position within Denbighshire. The post holder will work as part of the Learning Disability team based at Denbighshire, ensuring interfaces between the Local Authority and neighbouring services within the Division including community mental health teams, in patient services, as well as other statutory agencies and the voluntary sector. The post holder will deliver evidence based, safe, effective interventions and clinical care, following the nursing process.
The post holder will follow the service principles and strategy for people with a learning disability while delivering a safe, effective and high-quality specialist service in line with agreed standards. The post holder will have excellent communication skills and will be a visible and accessible member of the team; patients and their families can access for assistance, advice and support. The post holder will communicate and receive highly complex and sensitive information, analyse and interpret information, and act appropriately where there may be barriers to acceptance. The post holder will have a good understanding of the CHC process and be part of the development of pathways and the Maturity Matrix. Welsh language ability is desirable, and Welsh and/or English speakers are welcome to apply.
Responsibilities
* Deliver safe, effective and high-quality specialist services for people with a learning disability in line with standards and service principles.
* Communicate clearly with patients, families and colleagues; provide assistance, advice and support as a visible and accessible member of the team.
* Handle highly complex and sensitive information, analyse information, and respond appropriately to barriers to acceptance.
* Contribute to development of pathways and the CHC process.
* Collaborate with the Local Authority, community mental health teams, inpatient services, statutory agencies and the voluntary sector.
Qualifications and Experience
Qualifications and/or Knowledge
* RNLD Qualification
* Evidence of a degree level qualification or equivalent in line with NMC registration
* Evidence of postgraduate education and ongoing CPD
Experience
* Essential: Significant experience within the clinical speciality or a relevant nursing role
* Desirable: Experience of working in a multidisciplinary team; experience/knowledge of a senior nurse role
Aptitude and Abilities
* Ability to communicate and receive complex information
* Ability to diagnose problems and take corrective action
* Ability to analyse assessment results and translate them into relevant care packages; able to work autonomously and as part of a team
Desirable Skills
* Welsh language ability; IT skills; ability to manage resources
