Are you a confident, motivated Mental Health Nurse/Occupational Therapist or Social Worker with excellent communication skills and clinical decision-making abilities?

This role involves working in a fast-paced environment within a multidisciplinary team, providing high-quality assessment and home treatment services as alternatives to hospital admission for individuals in mental health crisis.
We seek warm, engaging individuals focused on client satisfaction.
Opportunities for secondment, fixed-term, or permanent contracts are available, with full-time and part-time options.
Applicants for secondment need prior approval from their current manager.
Main duties of the job
* Collaborate with inpatient wards to facilitate safe early discharges.
* Conduct first-line assessments of individuals experiencing acute mental health issues and act as gatekeepers to admissions.
* Rapidly assess vulnerable individuals to prevent hospital admission.
* Provide specialist crisis assessments focusing on “hospital at home”.
* Participate in team multidisciplinary meetings (MDMs).
