As a Bedded Care Coordinator, you will play a vital role in ensuring adults with care and support needs are placed in safe, suitable nursing and residential care settings, supporting positive outcomes for individuals across health and social care services.
You will be responsible for sourcing and coordinating the admission of suitable residential and nursing care options, working closely with hospitals, social work teams, and care providers.
The role involves building and maintaining strong relationships with providers, negotiating fees and promoting positive working practices. You will also contribute to the effective use of resources and support timely discharges from hospital, ensuring people receive the right care in the right place.
