Senior Community Staff Nurse – Workington, North Cumbria Integrated Care NHS Foundation Trust

Main Duties
Provide holistic, patient‑centred nursing care in the community and manage a complex caseload. Work with the primary health care team and other health and social care professionals to deliver high‑quality care, prevent hospital admission, facilitate discharge and promote independence.
Responsibilities
* Deliver nursing care covering weekdays, weekends, bank holidays and evenings.
* Support patients with complex needs and manage their clinical caseload.
* Provide clinical leadership and operational management for the community nurse team.
* Collaborate with primary care, social care and third‑sector partners to coordinate services.
* Maintain accurate documentation and communications with the wider team.
Qualifications
* Level 1 registered nursing qualification.
* Evidence of continuous professional development to degree level or equivalent experience.
* Post‑registration qualification in acute and chronic disease management.
* Nurse prescribing qualification.
* Specialist qualification in community nursing or equivalent.
Experience
* Use of initiative and effective prioritisation.
* Experience working as part of a team and motivating peers.
* Problem‑solving skills and critical decision making.
* Leadership and operational clinical management of a team.
* Knowledge of the changing health agenda and its impact on community nursing.
* Recent community nursing experience.
Knowledge
* Understanding of the extent of the role and scope of professional practice.
* Broad knowledge base of acute and chronic conditions.
* Knowledge of reflective practice and its application.
* Understanding of how health services are planned and delivered.
* Excellent communication and interpersonal skills.
* Ability to maintain confidentiality.
Personal Circumstances
* Able to travel independently.
* Able to work 24/7 and flexibly across a range of in‑patient and community settings.

Salary: £39,959 to £48,117 a year (pro rata).
