We are seeking to appoint dedicated and motivated Health Visitors to work within the High Peaks, North Dales and Glossop locality. The High Peaks, North Dales and Glossop 0‑19 Team has been awarded Gold accreditation for their work with Families and Communities. DCHS is a forward‑thinking, supportive Trust, who is a UNICEF Baby‑Friendly accredited organisation and achieved Outstanding CQC status in 2019. Working in a rural community you will find that there are lots of opportunities for developing public health work with communities and partner agencies, as well as delivering the core contacts within the Healthy Child Programme. The right candidates will hold the SCPHN qualification in Health Visiting and the V100/V150 and be innovative and dynamic team players who are looking for a new challenge. The base for this post is in either Glossop or New Mills however the team covers the whole of the North High Peak area. This is a rural location and requires travelling across the locality. Training days and meetings are sometimes in other parts of Derbyshire.
* Provide Health Visiting services to families in the community team caseload by delivering the Healthy Child Programme.
* Manage a defined active caseload and take responsibility for safeguarding children issues within that caseload.
* Have continuing responsibility for the assessment of individuals' needs and the development, implementation and evaluation of programmes of care.
* Prescribe according to assessed individual need from the Nurse Prescribing Formulary.
* Manage a team of Health Visiting support staff.
