Job Overview Please note for Welsh students, you must apply via Streamlining where all available jobs can be located An exciting opportunity has arisen to work as a District Nursing Discharge Support Nurse within Wrexham Maelor Hospital from 22nd February 2026. The role involves liaising with the Discharge Team, Ward Nursing Teams, Multi-disciplinary Team, Nurse Practitioners and other agencies to deliver a skilled, patient focused, efficient and effective service in the discharge process for patients from both Acute and Community Hospitals into the community setting. The post is 15 hours covering 2 days a week, to meet the service requirements. Main duties of the job The post-holder will work in an autonomous role alongside District Nursing Service within BCUHB in the discharge planning and safe discharge of patients following hospital admission. He/she will also be expected to work collaboratively with other agencies to plan care without direct supervision to ensure discharge planning is timely, coordinated and effective. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
