Are you a pharmacy technician looking for a new challenge? Do you have innovative ideas and are looking for the right team to impact on positive change? Are you looking to join a multidisciplinary (MDT) team where you can develop new skills as well as teach others?

An exciting opportunity has arisen for an experienced, enthusiastic, and highly motivated pharmacy technician to join our Integrated Placement of Care Hub (IPOCH) in a new pharmacy role to support direct patient care and be part of our community team within Central Cheshire Integrated Care Partnership.

They will be required to work across all CCICP geographical locations to support review of medication within patients' homes and in temporary residential care home settings. The postholder will be a registered pharmacy technician and an accuracy checker for checking dispensed prescriptions in accordance with Trust procedures.

The IPOCH service now supports a number of discharge services including our hospital discharge team, transfer of care hub, home care and community bed-based services. This team plays a significant role in supporting the flow out of hospital and providing admission avoidance support across community.

This role will require enthusiasm, self-motivation and commitment to developing new pharmacy roles within IPOCH, developing seamless pharmaceutical care for patients across traditional health and social care boundaries, and forging excellent communication between primary and secondary care. The postholder will:

1. Carry out medicines assessments to include communication of highly complex information in the home on a 1 to 1 basis with the patients/their representatives/other healthcare professionals if appropriate.
2. Work on an MDT level in both primary and secondary care to deliver patient focused care in the community and ensure that the individual patients' medicines support needs are met.
3. Support hospital discharge processes so that patients identified in hospital as having medicines support needs are followed up on discharge to ensure people with long term conditions can safely take or be administered medicines as prescribed in the home setting.
4. Review patients in hospital or temporary residential care home settings to develop care planning in relation to medication support needs.
5. Provide specialist training including, delivering training and competency assessing General Nursing Assistants (GNAs) and IPOCH registered nurses in medicines administration, medicines reconciliation and transcribing.
6. Provide specialist technical advice and undertake medicines reviews within clinical competence.
7. Be responsible for service development, audit and line management of pharmacy technicians working within IPOCH.

The role will encompass:

1. Medicines reconciliation for patients under the IPOCH GNA service.
2. Transcribing, and supporting the transcribing of, IPOCH GNA Medicines Administration Record charts.
3. Completing medication-related assessments to help the MDT determine if medication support is required in the community.
4. Providing patient education around medication, and assessment of medication related support aids.
5. Provide technical advice on medicines related safety and effectiveness issues and care plans to patients, practice staff, care home staff, GPs and other health professionals in health and social care.
6. Providing advice for patients, IPOCH registered nurses, and IPOCH GNAs on administration and handling of medicines.
7. Working with the MCHFT and CCICP Multidisciplinary Teams as well as pharmacy colleagues from across the wider system.
8. Supporting training and education of staff within the IPOCH service around medicines management.
9. Answering medicines-related queries within scope of competence.
10. Completing audits and participating in service improvement projects.
11. Ensuring safe practice and reducing medication associated risk.
12. Acting as the lead pharmacy technician for IPOCH, providing line management to other pharmacy technicians, and taking the lead on service development and continuous improvement within the service.

The successful candidate will be able to use their skills and knowledge to strive to deliver a high-quality pharmacy service within IPOCH. This post will be ideal for a pharmacy technician who is motivated by direct patient care, service improvement and innovation, wants to work collaboratively, and who wants to make a positive impact.

It will be expected that the successful candidate will hold a pharmacy technician clinical pharmacy diploma or will undertake within 2 years of starting the role.
