JOB SUMMARY/JOB PURPOSETo support and deliver the seamless provision of health care and positive mentalhealth promotion to men at HMP Cardiff under part one of the mental health (Wales)measure 2010.Working as part of a multi-disciplinary team this role has specific responsibility forliaising with the Primary healthcare team, secondary care and other appropriateagencies as well as providing assessment, triage and intervention for those in crisis,those with mild to moderate mental health needs or men with stable severe andenduring mental health conditions.KEY RELATIONSHIPS:? Main duties of the job
