Job summary
Manor Farm Medical Centre is looking for a salaried GP to join our busy practice. We are looking to cover 8 sessions per week and can be flexible for the right candidate with full time, part time or job sharing considered.
Working as part of our multidisciplinary team, you will have the opportunity to support and care for a diverse patient group. The role will include all general duties and responsibilities associated with a GP working within primary care.
Main duties of the job
1. Undertaking a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion;
2. Making professional, autonomous decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the organisation;
3. Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems;
4. Screening patients for disease risk factors and early signs of illness;
5. Developing care plans for health inconsultation with patients and in line with current practice disease management protocols;
6. Providing health education;
7. Recording clear consultation notes to agreed standards;
8. Collecting data for audit purposes and complying with QOF requirements;
9. Delivering enhanced services;
10. Compiling and issuing computer-generated acute and repeat prescriptions.
About us
Manor Farm Medical Centre is situated in a busy market town. We have approximately 7700 patients registered with us who are serviced by our main surgery in Swaffham and a branch surgery at Necton.
We are a forward thinking dispensing, research and training practice with 2 full time and 1 part time GP Partners, a part time salaried GP and a locum GP. We are a dedicated team working at the surgery providing a comprehensive service to our practice population. The wider clinical team comprises of 1 GP matron, 1 ANP, 3 practice nurses, 1 nurse associate, 2 highly skilled HCAs and a phlebotomist. Dispensary and reception staff provide a point of contact for patients and act as a focal point of communication between patients, doctors and other medical staff.
Our administration team is busy behind the scenes ensuring referrals, documents and registrations are processed and QOF targets are achieved. We are consistently performing well in research at the practice.
We are involved in teaching Y1 & 3 UEA MBBS students and the training of ST3s. The successful candidate with have the opportunity to become involved in teaching. We are an active participant in Swaffham and Downham PCN.

11. Fully qualified GP with GMC registration

14. Good understanding and experience of chronic disease management, and primary prevention and screening services;
15. Ability to take responsibility for your own career development, learning and performance and are responsible for ensuring that all your mandatory training is in date;

17. Commitment to life-long learning and audit to ensure evidence-based best practice;
