Job responsibilities
Key Responsibilities:
* Patient Observations:
* Conduct regular observations of patients, including monitoring vital signs, temperature, pulse, and respiration rates.
* Report any changes in patients' health status to the healthcare team and record on patients notes.
* Blood Sample Collection:
* Perform venipuncture and capillary blood sampling as required.
* Ensure samples are collected, labelled correctly, and transported to the laboratory in compliance with protocols.
* Patient Care:
* Make assessments of a patient's home surroundings, to offer social and mobility support.
* Discuss Future Care plans, support comprehensive geriatric plans, and personalised care plans.
* Administrative Tasks:
* Maintain accurate records of patient observations and care delivered.
* Ensure compliance with health and safety guidelines and organisational policies.
* Collaboration with Team Members:
* Work closely with the Non-Clinical Home Visiting Service Lead and other healthcare professionals within the service and Primary Care Network to coordinate patient care.
* Participate in team meetings and training as required.
You will be required to work from both our Hayling Island site and Emsworth site.

Qualifications
Essential
* Trained in phlebotomy.
Desirable
* Relevant certification or training in health care support (e.g., NVQ Level 2/3 in Health and Social Care).
Experience
Essential
* Previous experience as a Health Care Assistant or in a similar role is preferred.
* Strong interpersonal skills with a compassionate and patient-centered approach.
* Excellent communication and organisational skills.
* Ability to work independently and as part of a team.
* Valid driver's license and access to a vehicle for home visits.
