Responsibilities
* Provide high standard physiotherapy care to patients with neurological conditions and their carers/families in a community setting.
* Act as a resource for specialist advice to community therapy services and multidisciplinary teams.
* Perform comprehensive physiotherapy assessments for patients with complex physical and psychological conditions, provide clinical diagnosis and deliver individualized treatment programmes.
* Ensure all physiotherapy assessments are completed within agreed timescales.
* Support the B8a with day‑to‑day organisation of the community physiotherapy service and manage a designated caseload according to the service specification.
* Educate patients and carers in managing complex physiotherapy/rehabilitation needs within the community caseload.
* Collaborate with other agencies (specialist equipment services, social services, voluntary sector, private agencies) to fully integrate physiotherapy into patients’ care programmes where applicable.
* Hold full responsibility for own caseload of patients without direct supervision.
* Provide clinical supervision and support to less experienced physiotherapy staff, rehab assistants, technical instructors and students on placement, and support training and supervision of others in the multidisciplinary team.
