Job summary
The post holder is acting withintheir professional boundaries, will provide care for the presenting patientfrom initial history taking, clinical assessment, diagnosis, treatment andevaluation of care. They willdemonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills. Thepost holder will demonstrate critical thinking in the clinical decision-makingprocess. They will work collaborativelywith the general practice team to meet the needs of patients whether viatelephone, face to face or in the patients home or Care Home; supporting thedelivery of policy and procedures. In order to work at this level, NMCrequirements for advanced practice must be met, with experience as anIndependent Nurse Prescriber.
Main duties of the job
1. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
2. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
3. Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
4. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods.
5. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
6. Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
7. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
8. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
9. Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation where directed.
About us
We are a family orientated non judgemental Practice. As a member of our multi-disciplinary team you will work closely with our GPs & other members of our team dealing with on the day requests from patients having a mixture of Triage, Treat & visits within your rota.
Our extended team consists of GPs, Nurses, HCAs, Advanced Paramedic Practitioner, Pharmacist, physio, Mental Health worker, Social Prescribers.
We are also a training practice for GPs, Nurses & paramedics.
