Community Specialty Grade Doctor - Sheppey Community Hospital, SheernessAn exciting opportunity has arisen for a Dartford Gravesham and Swanley (DGS) Community Specialty Doctor to work across our 2-Hour Urgent Care Response Team; 40 bedded Community Virtual Ward and two nurse-led Intermediate Care Community Rehab Hospitals in providing medical care and advice to patients across these three services in Swale area.You will work as a member of our multidisciplinary team alongside the ANPs, an ECP, Lead Pharmacist, Ward Matrons, Specialist Nurses, and AHP leads, along with other clinicians, in delivering high quality of care to patients within their own homes or within their place of residence with the aim to facilitate early hospital discharge or avoidance of unnecessary hospital admissions. The three services above provide the following:40 bedded Community Virtual Ward with clearly defined patient pathways (frailty; respiratory and cardiac pathways).2-Hour Urgent Care Response Service to avoid unnecessary hospital admission.Two x 21-bedded Intermediate care inpatient rehab wards.This is a full-time position working Monday - Friday.We look forward to welcoming new colleagues; to find out more about the post please contact:Matt Tizora, Head of Operations for Swale Services via matt.tizora@hcrgcaregroup.com or matt.tizora@nhs.netMain ResponsibilitiesKey responsibilities for the post holder are to:Provide consultation and manage complex cases and act as an expert in the care they deliver with ultimate responsibility for the care of patients referred to them, even when care is carried out by other clinicians.Provide the highest standard of appropriate medical care in assessment and treatment and services to patients with various medical health needs, in line with national guidance including NICE and Quality and Outcomes Frameworks.Provide comprehensive holistic medical assessments and care to older people under their care.Respond to medical/health-related problems presented by patients including history taking, examination, diagnosis, investigation, treatment, and referral as appropriate.Ensure prescribing practice complies with policies and the formulary agreed by the Organisation considering national and local guidance. The Medication Policy must be always adhered to with appropriate risk assessments being carried out and documented as necessary.Involve patients in their care, wherever possible and appropriate.Participate in regular face to face or virtual MDT ward rounds and regular meetings to review patients on the virtual ward and community hospital wards.Work collaboratively with the Business Unit lead pharmacist to promote safe prescribing of medication across the services.Provide clinical care to patients in emergency situations, as necessary.Provide supervision and mentorship to student advanced nurse practitioners.Provide clinical supervision for non-medical prescribers.Provide teaching and training to others.A full list of responsibilities can be found in the attached job description.Ideal CandidateThe ideal candidate will have / will be:UK medical experienceA Primary medical qualificationExperience in general internal medicine or geriatric medicineWilling to work towards Diploma in older people’s medicineCompleted GP trainingPost-graduate experience as a doctor in the NHSMS office IT skills and system knowledgeCommitment and ability to deliver a high standard of clinical careSelf-motivated and pro-activeExcellent communication skillsDemonstrates skills in written and spoken English, adequate to enable the post holder to carry out the role effectivelyYou must be a driver with a full UK Driving Licence and access to a vehicle for work purposes.About the CompanyWe change lives by transforming health and care.Established in 2006, we are one of the UK's leading independent providers of community health and care services, working with health and care commissioners and communities to transform services with a focus on experience, efficiency and improved outcomes.
