Are you a passionate and dedicated GP looking to make a realdifference in a patient-centred, forward-thinking practice? Were excited towelcome a new GP to our growing team at Sarum Health Group, where yourwellbeing, development, and work-life balance matter.

This General Practitioner role will involveworking alongside our multi-disciplinary team which consists of a GP Partners,Advanced Clinical Practitioner Partner and Pharmacist Partner. They are allsupported by Advanced Clinical Practitioners who lead on acute same day care,alongside the Home Visiting Team, First Contact Physio, Pharmacists, PracticeNurses, HCAs, Dispensers and Admin staff. We also have Social Prescribers who work as PCN staff dealingwith the Mental Health of our patients.

The successful General Practitionerwill have support from in-house team for prescription queries, document management, triage and will work asan autonomous practitioner, diagnosing, managing, prescribing and dischargingpatients with illness or injury across all age groups including young children.This may be by a mix of face-to-face consultation, telephone or video consultation.

Main duties of the job

As a General Practitioner working within our GP surgery, youwill will be a key member of our clinical team, delivering safe, effective and high-quality primary care services to our registered population. Yourresponsibilities will include:

-Clinical Care;

-Prescribing and Medicines Management;

-Chronic Disease and Long-Term Condition Management;

-Health Promotion and Preventive Care;

-Referrals and Coordination of Care;

-Clinical Administration;

-Team Working, Collaboration and Communication;

-Clinical Governance, Quality and Development;

-Home Visits (as required);

-Teaching and Mentorship (Optional).

On-site dispensary located in at our branch sites inLarkhill, Sixpenny Handley and Broad Chalke.

SystmOne Web clinical system, also using Cinapsis, AccuRX,SystmConnect, T-Quest and TeamNet.

Full complement administration team who oversee letterprocessing, insurance reports, solicitor reports and all other non-NHS works,safeguarding reports and referrals.

Enhanced hours as part of our PCN network of practices

1 intermediate care bed which we provide a weekly on-siteclinic with limited number of patients.

Nurse-led chronic disease clinics.

Job responsibilities

Core Principles:

To support and participate in initiatives toensure a safe and healthy environment for all practice users. This includesfollowing procedures to ensure the control of potential hazards to health andsafety.

To contribute to a culture of continuousimprovement

To deal with complaints promptly and accordingto practice policy

To perform duties to standards required by thepractice in accordance with quality assurance.

Primary Responsibilities:

The following are the coreresponsibilities of the Salaried GP. There may be, on occasion, a requirementto carry out other tasks; this will be dependent upon factors such as workloadand staffing levels:

The delivery of highly effective medical careto the entitled population

The provision of services commensurate withthe GMS contract

Generic prescribing adhering to local andnational guidance

Effective management of long-term conditions

Processing of administration in a timelymanner, including referrals, repeat prescription requests and other associatedadministrative tasks

On a rotational basis, undertake telephonetriage and duty doctor roles

Maintain accurate clinical records inconjunction with good practice, policy and guidance

Work collaboratively, accepting an equal shareof the practice workload

Adhere to best practice recommended throughclinical guidelines and the audit process

Contribute to the successful implementation ofcontinuous improvement and quality initiatives within the practice

Accept delegated responsibility for a specificarea (or areas) or the QOF

Attend and contribute effectively to practicemeetings as required

Contribute effective to the development andmaintenance of the practice including clinical governance and training

Ensure compliance with the appraisal process

Prepare and complete the revalidation process

Commit to self-learning and instil an ethos ofcontinuing professional development across the practice team

Support the training of medical students fromall clinical disciplines

Support the partners in achieving thestrategic aims of the practice, making recommendations to enhance income andreduce expenditure

Review and always adhere to practice protocolsand policies

Encourage collaborative working, liaising withall staff regularly, always promoting a culture of continuous improvement.

Secondary Responsibilities:

In addition to the primaryresponsibilities, the salaried GP may be requested to.

Participate in practice audits as requested bythe audit lead

Participate in local initiatives to enhanceservice delivery and patient care

Participate in the review of significant andnear-miss events applying a structured approach i.e., root cause analysis (RCA)

Confidentiality:

While seeking treatment, patients entrust uswith, or allow us to gather, sensitive information in relation to their healthand other matters.They do so inconfidence and have the right to expect that staff will respect their privacyand act appropriately.

In the performance of the duties outlined inthis job description, the post-holder may have access to confidentialinformation relating to patients and their careers, practice staff and otherhealthcare workers.

They may also haveaccess to information relating to the practice as a business organisation. All such information from any source is to beregarded as strictly confidential.

Information relating to patients, carers,colleagues, other healthcare workers or the business of the practice may onlybe divulged to authorised persons in accordance with the practice policies andprocedures relating to confidentiality and the protection of personal andsensitive data.

Health and safety:

The post-holder must complyat all times with the Practices Health and Safety Policies, in particular byfollowing agreed safe working procedures and reporting incidents by followingthe Practices incident reporting procedure.

Learning and development:

The effective use of training and developmentis fundamental to ensuring that all staff are equipped with the appropriateskills, knowledge, attitude and competences to perform their role. All staffwill be required to partake and complete mandatory training as directed by thetraining coordinator, as well as participating in the practice trainingprogramme.

Staff will also be permitted (subject toapproval) to undertake external training courses which will enhance theirknowledge and skills, progress their career and ultimately enable them toimprove processes and service delivery.

The post-holder shouldrecognize the importance of effective communication within the team and willstrive to:

Communicate effectively with other teammembers.

Communicate effectively with patients andcarers.

Recognise peoples needs for alternativemethods of communication and respond accordingly.


Person Specification


Qualifications

* GMC registered with a license to practise;
* On the NHS England Performers List.


Experience

* Experience working in General Practice/Primary Care setting;
* Experience of working to achieve standards within the Quality and Outcome Framework (QoF) Working autonomously in an General Practitioner role.


Good interpersonal and communication skills

* Excellent communication, teamwork, and patient care skills;
* Enthusiastic, collaborative and keen to contribute to a positive team culture.
