An exciting opportunity has arisen for an experienced Advanced Nurse Practitioner to join our unique, highly skilled VAWAS (Vulnerable Adult Wrap Around Service) team.The role of the VAWAS ANP requires an experienced nurse who, actingwithin her / his own professional boundary, will provide care for patientswithin all the Nursing and Care homes in the localities, and all otherpatients, within their own homes who have been referred into the scheme by GPpractices within the above boundaries.The ANP will work collaboratively with the general practiceteam to meet the needs of patients, supporting the delivery of policy andprocedures.


Main duties of the job

Assess patients presenting with acute illness livingwithin the Nursing and Care Homes within the locality who are at risk of inappropriatehospital admission.Diagnose, plan, implement and evaluatetreatment, interventions and care for all patients presenting with anundifferentiated diagnosis and those with complex needs. Clinically examine and assess patient needs froma physiological, psychological and sociological perspective, and plan clinicalcare accordingly.Proactively identify, diagnose and managetreatment plans for patients at risk of developing a long-term condition (asappropriate). Diagnose and manage both acute and chronicconditions, integrating both drug- and non-drug-based treatment methods into amanagement plan. Refer patients to other medical specialities forassessment if deemed appropriate.Refer patients to other medical specialities forassessment if deemed appropriate. Prescribe and review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols and formularies. Work with patients and their carers in order tosupport compliance with and adherence to prescribed treatments.


About us

Jubilee Medical is an established GP practice based in Newton Aycliffe with a branch site in Shildon. The practice is passionate about providing additional services to our local community, currently offering consultant led Paediatric and ENT clinics, out of hours access and home visits (including weekends and bank holidays) to all patients registered within our Primary Care Network. We also have a dedicated team of Advanced Nurse Practitioners supporting our local care homes ensuring our elderly population receive the appropriate clinical care and monitoring on a weekly basis. The practice has been operating as a Covid Vaccination Site since December 2020.


Job responsibilities

The role of ANP requires an experienced nurse who, actingwithin her / his own professional boundary, will provide care for patientswithin all the Nursing and Care homes in the localities, and all otherpatients, within their own homes who have been referred into the scheme by GPpractices within the above boundaries.

Acting as an autonomous practitioner working independently,and in conjunction with, other health care professionals to assess,investigate, diagnose, treat and evaluate all patients seen, includingreferring into other services wherever or whenever required. The post holderwill demonstrate safe, clinical decision-making and will provide expertprofessional advice to patients, carers and colleagues ensuring maintenance ofclinical excellence.

The ANP will work collaboratively with the general practiceteam to meet the needs of patients. Theywill provide education and training to other staff and students, and provideleadership as required.

* Able to travel between nursing and care homes as and when required.


Qualifications and professional registration

* Registered Nurse with Nursing and Midwifery Council
* Post graduate diploma or degree for Advanced Practice Qualification
* Qualified Independent Nurse Prescriber on the NMC register
* Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
* Experience of using SystmOne
* Experience of elderly care


Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Depending on experience21 hours per week pay between £28.03 - £31.56
