About the Role
Are you passionate about working within Adult Social Care and making a difference to the lives of others? Could this be the next step in your social work career?
An exciting opportunity has arisen to work across Adult Social Care (Adult Duty Team) and Housing (Housing Solutions) to support citizens who are rough sleeping or homeless and at risk of rough sleeping and have Adult Social Care eligible needs. In this new and innovative role you will be working in close partnership across both services to ensure individuals receive a connected and coordinated response, receive the right assessments at the right time and are supported to navigate through appropriate housing pathways and link in with relevant agencies and partners.
Working within the parameters of the Care Act and using a strength-based and person-centred approach, you will be managed and based within the Adult Duty Team but will be co-located within Housing Solutions where you will work closely with the team's Early Intervention Officers and Navigators. You will coordinate Care Act assessments alongside colleagues completing statutory housing assessments to provide more efficient and improved outcomes for people to access the most appropriate accommodation options. You will undertake associated work with citizens including the commissioning of support, Mental Capacity Act assessments and BI decisions where required and make safeguarding enquiries and assess and manage risk.
We are looking for the right person for the role wherever you are in the Social Work Career Pathway detailed in the salary outline above.
About You
The ideal candidate will have:
1. Professional Social Work Qualification and registered with Social Work England
2. Direct experience of working with adults and understanding of homelessness and wider impacting issues
3. Strong assessment skills, including assessment of risk and mental capacity and ability to competently manage a complex caseload
4. Evidence of working successfully in partnership across different sectors, building and maintaining good working relationships
- A DBS enhanced check for a regulated activity is required for this post.
