Are you a highly motivated, experienced Advanced Clinical Practitioner, looking for a new challenge? Adult Mental Health Services are looking for someone who possesses confidence, enthusiasm, initiative and a positive approach to teamwork but most importantly has a genuine passion for patient care.

AMH have exciting plans to modernise our workforce to meet current and future challenges and provide a new career pathway for non-medical practitioners through the introduction of Advanced Clinical Practitioners into our community services.

This role is based in the Broxtowe & Hucknall LMHT. It is anticipated that this role will develop over time in response to changes in service delivery and therefore we need clinicians who are energetic, eager to develop and are forward thinking with the potential to adapt to change and meet new challenges.

We will ensure we support you at every level to enable you to develop the role within AMH services.

Alternatively, we would consider appointing a Trainee Advanced Clinical Practitioner (tACP) to develop specialist knowledge and skills to provide healthcare autonomously to patients in their speciality through an established ACP training pathway.


Responsibilities

* Demonstrate safe, senior clinical decision‑making by autonomously assessing patients presenting to the inpatient service including initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
* Demonstrate critical thinking in clinical decision‑making and work collaboratively with the MDT to meet the needs of patients.
* Guide junior medical staff and nursing staff in relation to relevant protocols, procedures and clinical pathways.
* Fundamental in the development of this new role and initiate, manage and drive innovative change for the benefit of patient care in line with Trust and local procedures.
* Provide mentorship to trainee ACPs within the directorate.
* Held 1st on‑call duty on a 3‑month rota basis as part of this role.
* Carry out systemic and psychosocial examination, patient history, make diagnostic decisions based on consultation, history, presentation, pathology and imaging results.
* Initiate, prescribe and monitor the impact and outcome of treatments and medication including efficacy, side‑effects, contraindications and risk.
* Devise an individual clinical management plan that includes all aspects of multi‑disciplinary care.
* Screen patients for early signs of disease and risk factors and relapse indicators.
* Run independent out‑patient clinics seeing new and review individuals as required.


Qualifications and Professional Experience

* Accredited ACP Masters‑level qualification.
* Accountable for independent clinical assessment, formulation, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions.
* Practice in compliance with respective code of professional conduct and within scope of practice, being responsible and accountable for decisions, actions and omissions.
* Demonstrate a critical understanding of broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice.
* Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self‑awareness, emotional intelligence and openness to change.
* Work in partnership with individuals, families and carers using a range of assessment methods (history‑taking, holistic assessment, risk factors, mental health assessments).
* Demonstrate effective communication skills supporting people in making decisions, planning care or seeking to make positive changes, through person‑centred approaches in health and care.
* Use expertise and decision‑making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence‑based judgements and/or diagnoses.
* Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.
* Exercise professional judgement to manage risk appropriately, especially where complex and unpredictable events arise and support teams to do likewise to ensure safety of individuals, families and carers.
* Work collaboratively with an appropriate range of multi‑agency and inter‑professional resources, developing, maintaining and evaluating links to manage risk and issues across organisations and settings.
* Act as a clinical role model/advocate for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies and networks.
* Evidence underpinning subject‑specific competencies (knowledge, skills and behaviours) relevant to the role setting and scope, and demonstrate application of the capabilities to these, in an approach that is appropriate to the individual role, setting and scope.

Applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment.
