Job Purpose
The post holder(s) will work as part of the clinical pharmacy team in University Hospital Monklands providing clinical pharmacy services across a range of specialties including Infectious Diseases, Renal Medicine, Surgery and Anaesthetics, and General Medicine. You will work as part of a multidisciplinary team to ensure safe, effective, and evidence-based use of medicines, while supporting education, audit, and service development. You will be working across the 4 pillars of advanced pharmacy practice and so undertaking the RPS core advanced curriculum is highly encouraged.
The Role

Additional Information Specific To The Role
Delivery of high-quality clinical pharmacy services to designated wards and departments including:
* Surgery & Anaesthetics: Critical care training, surgical receiving, and urology wards supporting perioperative prescribing and working with the MDT.
* Medical Rotations: Acute medicine, medical high dependency, frailty, respiratory, cardiology, gastroenterology and endocrinology. Opportunities available for training to provide outpatient clinic support.
* Infectious Diseases: Antimicrobial stewardship, supporting patients with blood borne viruses, supporting NHS Lanarkshire OPAT service.
* Renal: Supporting dialysis patients, working with the renal pharmacy team, supporting renal anaemia clinics.
You will participate in consultant ward rounds and multidisciplinary team meetings in addition to conducting patient specific medication reviews. You will supervise and mentor junior pharmacists, foundation trainee pharmacists, and pharmacy students. The post provides the opportunity for delivering teaching sessions (formal and informal) to medical, nursing, and pharmacy staff. You will be required to lead or contribute to clinical guideline development, clinical audits, quality improvement projects, and service evaluations; support implementation of local and national guidelines and participate in risk management, incident reporting, and medicines safety initiatives.
Operational Duties Include
* Supporting dispensary and clinical screening duties as required.
* Contributing to weekend and on-call rotas.

What we'll need you to bring
Masters of Pharmacy Degree
Registered member of General Pharmaceutical Council
Independent prescribing qualification or must work towards
Evidence of postgraduate training and professional development in clinical pharmacy practice e.g. Post Registration Foundation Framework/ 1st year clinical diploma
Relevant post registration experience in clinical pharmacy.
Ability to effectively monitor patients’ therapeutic regimens in order to ensure effectiveness and reduce toxicity
Ability to work effectively as part of a team, in a pharmacy and multi-disciplinary environment.
Commitment to revalidation as per directions of General Pharmaceutical Council
Good verbal and written communication skills
Self-motivation
Participation in weekends and public holiday service
Participation in pharmacy emergency duty service
Willingness to contribute to the general working of the pharmacy department
Please note for all vacancies where a driving licence is required this must be a full UK/EU/EEA licence.
It would be great if you also have
Demonstrable post registration experience
Relevant post registration experience in hospital pharmacy
Demonstrable experience in providing a clinical pharmacy service to a defined patient group
Experience in patient information/education
Experience in supervising staff
Clinical pharmacy / care planning
Proven project/practice research/ audit skills

1x Fixed Term or Secondment for 12 months

Please note that the fixed term post is for 12 months. Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post.
Location and Working Pattern
This role will be based in University Hospital Monklands
The working pattern for this role Mon-Fri; 9:00-5:00
