We are a friendly and supportivetraining practice located on the outskirts of Halifax, West Yorkshire and wehave a very exciting opportunity for a motivated, committed and experienced CareNavigator to join the team for 25 hours per week.
Main duties of the job
Maintaining andmonitoring the practice appointment system
Process personal,telephone and e-requests for appointments
Answer incoming phonecalls, transferring calls or dealing with the callers request appropriately
Signpost patients tothe correct service
Initiating contact withand responding to, requests from patients, team members and external agencies
Data entry of new andtemporary registrations and relevant patient information as required
Input data into thepatients healthcare records as necessary
Direct requests forinformation i.e. SAR, insurance / solicitors letters and DVLA forms to theadministrative team
Manage all queries asnecessary in an efficient manner
Maintain a clean, tidy,effective working area at all times
Monitor and maintainthe reception area and notice boards
Support all clinicalstaff with general tasks as requested
About us
The team includes six GPPartners, three Salaried Gps, four Advanced Clinical Practitioners who aresupported by a full Nursing Team comprising of four Practice Nurses, oneNursing Associate, one Healthcare Assistant, two Phlebotomists as well asPharmacy Prescribers. The practice also has First Contact Physiotherapy andPodiatry services running from the practice.

We are looking for a motivated Care Navigator who is passionate about providing high quality healthcare and tackling the rolein new and exciting ways. We want you to come and work alongside an alreadymotivated and inspiring practice team of like-minded and varied professionals.
The post holder will be expectedto undertake all the normal duties and responsibilities associated with aCare Navigatorworking within primary care.
