Advanced Clinical Practitioner - Living Well & Care Coordination Centres
* Hours: 37.5 hours per week (part-time hours considered)
* Location: Grove House, St Albans
* Salary: Rennie Grove Peace Band 8a - £51,971 - £58,496 per annum (pro rata)

The role of Advanced Clinical Practitioner - Living Well & Care Coordination Centres is in place to improve the clinical practice and patient outcomes across RGP Living Well and Care Coordination Centres services, through the creation of a culture of learning, quality improvement and innovation. Using advanced clinical expertise and extensive knowledge of the palliative care sector to deliver direct patient care and support colleagues to ensure best practice standards are maintained and improved patient outcomes.
You will therefore need to demonstrate your abilities in the following areas:
* Lead and support the operational delivery of the RGP strategy into day-to-day clinical practice, ensuring the agreed annual Patient Services work plan timescales are met.
* Deliver evidenced based practice and improved patient/carer outcomes across the end-to-end patient pathway.
* Monitor the delivery of safe, responsive patient care, proactively leading on multidisciplinary care planning meetings/case load reviews to ensure care is effectively co-ordinated and delivered.
* Embed the RGP Quality and Assurance framework into day-to-day practice in collaboration with the governance team, ensuring clinical policies & best practice guidelines are in place and audit findings are implemented.
* Proactively manage patient related accidents and incidences and embedding an open transparent culture that support continuous improvement and when required recommendations to changes in practice/ processes.
* Provide oversight and direction on the development needs of the wider clinical workforce, working with the Practice Educators to ensure the required internal and external training, programmes and resilient supervision practices are in place.
* Working with the Learning & Development team make Palliative Care an attractive place to work, ensuring a robust induction and mentorship pathway. With a steer on the development of external multi-professional student / career placements.
* As an expert practitioner, mentor and coach to enhance our nursing/ AHP’s clinical decision-making confidence and competence, ensuring effective use of our clinical workforce's knowledge and skills to proactively respond to our patient/carers needs.
* Keep abreast of both national and local palliative care initiatives. Ensuring any service redesign/ evaluation developments drive maximum efficiency and improve patient experiences.
* Working collaboratively, influence and seek opportunities to further enhance wider RGP patient earlier diagnosis pathways. With a particular focus of raising awareness and engagement with other NHS/ Social Care long-term conditions leads, creating joint patient initiatives that avoid any unnecessary duplication.
* Working with the Business, Operations & Performance Managers, Medical Consultants and other external partners explore where the ACP leadership and expertise can be best utilised to the best effect.
* Be research proactive.
* As the nominated Medical Devices Safety Officer (MDOS) lead, chair and lead on the designated Patient Safety interest group, reporting to the wider RGP Patient services Safety group.
