Hours: Monday to Friday, 8:30 AM 4:30 PM or 9:30 AM 5:30 PM (On-call duties as required)

Main duties of the job
As a Specialist Clinical Pharmacist, the post holder will play a pivotal role within the pharmacy team, contributing to the delivery of a safe, effective, and patient-centred pharmacy service across the Trust.
Key Responsibilities include:
a) Clinical Ward-Based Services:Delivering a high-quality, patient-focused clinical pharmacy service on wards, ensuring the safe and effective use of medicines.
b) Supporting Timely and Safe Discharges:Facilitating early discharge and reducing hospital length of stay by:Optimising patients drug therapy.Providing tailored education on medicine use, including support with self-medication and assessing the need for adherence aids.Offering comprehensive counselling to patients about their medications prior to leave and discharge.Ensuring continuity of care through robust discharge planning and communication with community healthcare providers.
c) General Pharmacy Duties:Participating in wider clinical pharmacy and departmental responsibilities as required, supporting the overall objectives of the Trusts pharmacy service.
