Due to the current post-holder moving onto pastures new, an exciting opportunity has arisen to join the dynamic and growing team at Nuneaton & Bedworth Primary Care Network (PCN).
This role will work as part of our Personalised Care Team to provide administrative support in terms of processing referrals into the team, maintaining the referrals/marketplace database and supporting the Senior Team members.
Main duties of the job
We have recently brought our Social Prescribers, Health & Wellbeing Coaches (H&WBC) and Care Coordinators in-house and are keen to develop closer working relationships between these staff and our Mental Health team under the umbrella of Personalised Care, to support the needs of our patients and member practices.
You will maintain the recently implemented 'Joy' system which is used to record, allocate and manage referrals to our Social Prescribing and H&WBC teams, monitor Lone Working for the staff when they go to off-site to see patients and act as co-ordinator for the Personalised Care Team, so need to be good at communicating and enjoy working as part of a team.
One of the services that we provide for our patients is a 'Walk and Talk' initiative. Part of your role will be to work with the Health & Wellbeing Coach to coordinate patient numbers and support for the group meetings.
The Personalised Care Team meets every two months to share information and provide peer support/case reviews. You will work with the Team Leaders to organise these meetings, send out invitations, create agendas and produce the minutes.
You will also be required to support 2 meetings per month for the Warwickshire North Primary Care Networks.

Nuneaton & Bedworth PCN is a large organisation covering 16 GP practices and roughly 120,000 patients.
We have more than 50 Additional Roles staff working across the practices in a number of different roles, which includes the Personalised Care Team.
We are really invested in our workforce, including the Personalised Care Team. We have recently expanded the Care Coordinator team from 3 to 7, the Social Prescribing team is expanding from 5 to 7 members and we have plans to recruit an additional Health & Wellbeing Coach over the coming weeks.

Monitor and allocate referrals coming into the Team via the Joy system.

Work with the Health & Wellbeing Coach(s) to support the Walk& Talk project.
Maintain the Marketplace within the Joy system to ensure thatorganisation details remain up to date for referral pathways for patients.
Inductnew starters in use of the Joy system.
Provideadministrative support to the Warwickshire North Primary Care Board and WarwickshireNorth Clinical Directors monthly meetings.This will include co-ordinating the Agendas, taking minutes and maintainingaction trackers.
