A Care Coordinatorplays an important part within the surgery, They need to have a high level of administrative experience, they also need to be caring, dedicated,and reliable and enjoy working with a wide range of people. They willparticipate fully in the running of the Practice within the scope of specifiedadministration duties detailed in the job description. You will need to provide an efficient servicefor the clinical staff and Patients. Being responsible to the Practice Managerand conducting oneself in a pleasant and business-like manner. It the successful individual with be involved in exciting new income generated projects and support other administrative role within the practice.


Main duties of the job

Manage & Support annual leave andarrange cover where necessary

Manage staff sick leave andarrange cover where necessary

Oversee the day-to-dayoperation during working hours of the reception team to ensure that

staffachieve their primary responsibilities, and ensuring that the wait on thepractice

telephone lines is not excessive at any one time.

Support and monitor staff inthe use of IT; organise, oversee and evaluate IT training where

appropriate.

Maintain a good knowledge ofthe clinical systems and provide training where required to staff

and patientsto enhance patient care.

Coordinate and support managementof the daily distributions of blood results.

Coordinate and support managementof 111 & OOH reports

Coordinate and support managementof the Breast Screening DNA follow ups.

OverseeQOF and generate reports on health inequalities.

We are an expanding 4 Partner PMSteaching practice with a growing patient population of 12,300 using EMIS webclinical system. We highly value bothour patients and our staff.

Keyresponsibilities

Manage staff annual leave andarrange cover where necessary to ensure that a minimum level of staffing isavailable to ensure that patient care is not impacted

Manage staff sick leave andarrange cover where necessary to ensure that a minimum level of staffing isavailable so that patient care is not impacted.

Coordinate general tasks thatget generated from GPs

Coordinate and support managementof the Breast Screening DNA follow ups.

OverseeQOF and generate reports on health inequalities.

ManageWeight Management for health promotion targeting vulnerable patient cohorts.

Manage3LTC Project

OverseeLearning Disability reviews

ManageCare Home Contract liaise with Care Home Managers, arrange regular reviewmeetings and transcribe minutes.

Undertake continual personaland professional development, taking an active part in reviewing anddeveloping the role and responsibilities, and provide evidence of learning areawhen required.

Adhere to organisational policies and procedures,including confidentiality, safeguarding, lone working, information governance,equality, diversity and inclusion training and health and safety.

Establish strong working relationships with GPs andpractice teams and work collaboratively with others.

Demonstrate a flexible attitude and be prepared tocarry out other duties as may be reasonably required from time to time withinthe general character of the post or the level of responsibility of the role,ensuring that work is delivered in a timely and effective manner.

Identify opportunities and gaps in the service andprovide feedback to continually improve the service and contribute to businessplanning.

Contribute to the wider aims and objectives of the PCNto improve and support primary care.


Person Specification


Knowledge & Skills

* *Understanding of, and commitment to, equality, diversity and inclusion
* *Strong organisational skills, including planning, prioritising, time management and record keeping
* *Ability to recognise and work within limits of competence and seek advice when needed
* *Basic knowledge of long-term conditions
* and the complexities involved: medical, physical, emotional and social

* *Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers
* *Knowledge of how the NHS works, including primary care and PCNs

Qualifications

* *Educated to GCSE level
* *Undertake all relevant mandatory training
* *Demonstrable commitment to professional and personal development is enrolled in, undertaking or qualified from appropriate training as set out in the core curriculum by the Personalised Care Institute
* *Proficient in MS Office and web-based services
* *NVQ Level 3 in Health and Social care or Customer Services qualifications or working towards


Experience

* *Experience of working within multi professional team environments
* *Experience of data collection and using tools to measure the impact of services
* *Experience of working directly in a care coordinator role, adult health and social care, learning support or public health /
* health improvement
* *Experience of working in health, social care and other support roles in direct
* contact with people, families or carers (in a paid or voluntary capacity)
* *Experience of supporting people, their families and carers in a related role
* *Experience or training in personalised care
* and support planning
* *Experience of working with elderly or vulnerable people, complying with best practice and relevant legislation
