Job summary We are looking for an existing General Practitioner to work with the Community and Older Peoples Medicine Services, two sessions a week, to support a clinical caseload of patients undergoing rehabilitation alongside a multidisciplinary team to promote recovery of function and a return to independent living where possible. Main duties of the job The postholder will work as an integral member of the intermediate care team as the named clinician for a cohort of 10-15 patients. They will be expected to oversee and coordinate a Comprehensive Geriatric Assessment for patients who have either stepped up from the community to avoid acute hospital admission or are clinically stable following and acute admission. It is expected that the post holder will provide clinical sessions within the intermediate care unit/rehabilitation ward for 2 sessions per week. Ideally sessions will be worked in the morning on a Monday and Thursday.
