Band 3 - Rehabilitation Assistant
An exciting opportunity has arisen to join the Rehabilitation and Frailty Team. We are looking for an experienced and motivated health care worker who is passionate about providing high standard of patient centred care. You will be working closely with community nurses, community matrons/ care coordinators, physiotherapists, occupational therapists, and rehabilitation assistants who are committed to maintaining independence, promoting self-care and assisting patients to achieve their goals.
Responsibilities
* The successful candidate will be hardworking, reliable and able to use their initiative and work flexibly to respond to service needs.
* Be part of a friendly and approachable team, and be well supported by colleagues and line manager.
* Work as a Rehabilitation Assistant within the Rehabilitation and Frailty Team as part of a multidisciplinary team providing care and see a wide variety of patients predominately with physical conditions at home.
* Be responsible for a delegated caseload as directed by qualified staff.
Working pattern and location
The Rehabilitation and Frailty Team currently operates from 07:00 to 22:00 every day, and Rehabilitation Assistant core hours are between 09:00 and 17:00 Monday to Friday. Weekends and Bank Holidays are covered by Rehabilitation Assistants on a rota. We cover Basildon, Wickford, Billericay, Brentwood and Thurrock areas.
Organisational context
Starting with NELFT: NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters join the Trust on the first Monday of each month and undertake a comprehensive induction of up to two weeks which includes mandatory training, systems training and the allocation of equipment. New starters will have the opportunity to meet the executive team, senior managers and attend drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
Probationary period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
Compensation and benefits
High Cost Area Supplement - Fringe: This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,303 and a maximum payment of £2,198 per annum, pro rata for part time).
