St Lukes Hospice is looking for an Integrated Care Facilitator with experience in health and social care, passionate about improving the lives of our palliative care patients and their significant others.
About The Role
As an Integrated Care Facilitator, you will leverage your knowledge of the health and social care system to support clinical colleagues within the Hospice, primary care, and wider healthcare providers. Your role involves working directly with patients and their significant others in their communities, understanding local initiatives that can support them creatively.
You will be instrumental in developing and evaluating this role, with your insights helping us excel. We recognize the challenges in current health and social care provision and seek someone who can maximize support and choice for our patients, from referral to end of life.
The role requires initiative and teamwork, supporting our three community hubs.
About You
We value your core values most. Experience in clinical settings or specialist palliative care is advantageous but not essential; training will be provided. Knowledge of care provision across Plymouth, South Hams, and West Devon is a plus.
