About the role

An exciting opportunity has arisen within the Children’s Community Nursing Team for x 2, Band 5 Children's Community Nurses - Permanent - Full Time.

The Children’s Community Nursing Team are a friendly, welcoming Team of Nurses who are passionate about meeting the health needs of children & young adults referred into the Service.

The Children’s Community Nursing Team offer specialist nursing support to families who have children or young people with nursing or medical needs in their homes or other community setting such as schools and nurseries. The Team provides support to specialist tertiary teams in providing nursing cares away from a hospital setting.

The role will include supporting with Nutrition and Enteral feeding, Oncology/chemotherapy treatment and support, Rheumatology care and support, Respiratory support, Wound care, Parental/carer training, Symptom management, Diagnosis and case management, Liaison (local & regional), Equipment provision/home delivery equipment, facilitate seamless care from tertiary & secondary care to primary care.

The role is Community based; you will need to hold a full driving licence plus have access to a car. The working hours are:

Monday Friday 09:00am 17:00pm, On Call over a weekend/Bank Holidays.

Main duties

The post holder is required to provide effective patient centred care by performing individual assessment of care needs, developing, implementing and evaluating programmes of care that meet the needs of the patient and their families/carers referred into the Service. The role is multidimensional and requires the post holder to integrate and fulfil all the identified dimensions. As a registered nurse all activities provide opportunities for acting as a role model for staff. Following their preceptorship period the registered nurse is expected to carry out all relevant forms of care without direct supervision. The registered nurse will also undertake the management of a defined caseload indirectly supervised by a Senior Community Nurse.

Essential person specification requirements

Specifications - Applicants will need to have been qualified for a minimum of 6 months - a year. All applicants will need to be competent in the following competencies:

Tracheostomy - Desirable

Portacaths, CVL, Broviac Lines Line cares plus obtaining bloods - Desirable

Ventilation Desirable

Nasogastric Tubes, Gastrostomy, Jejunostomy

Wound Cares Desirable

IM & SC Injections

End of Life Care Desirable

Seizures

Medicines Management

Intravenous Medication

Have access to a car for work purposes plus a valid UK Driving Licence
