Essential criteria
1. Educated to degree level in Nursing (RGN) and current NMC registration
2. Relevant post-qualification experience in acute or community nursing settings
3. Experience of completing CHC assessments (Checklist and DST)
Desirable criteria
4. Best Interest Decision Making / MCA / DoLS training
5. Experience of partnership working with Social Services and external agencies
6. Understanding of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care

7. Experience of working in a multi-disciplinary team
8. Experience of supporting patients with complex or challenging needs (frailty, delirium, non-weight bearing, behaviours that challenge)
Desirable criteria
9. Strong written communication and report writing skills, with the ability to produce clear, accurate, and comprehensive clinical reports and documentation
10. Ability to manage and facilitate difficult and sensitive conversations with patients, families, and partner agencies, demonstrating empathy and professionalism.
