Are you an experienced community mental health nurse, social worker or occupational therapist who would like the opportunity to be part of a innovative integrated Community Mental Health Service? Are you looking for an opportunity to be part of a service model that is working in partnership with local services to meet the needs of those presenting with complex mental health needs together?

Telford and Wrekin Community Mental Health service provide assessment and evidence based interventions for people who presenting with complex mental health difficulties that are significantly impacting on their daily life alongside our partner organisations.

We are looking for a practitioner to the join our team to work alongside our primary care practitioners and wider partners to support the triage and assessment of those being referred into our service.

You will need to encourage effective engagement of this cohort of people, utilising excellent communication skills, whilst maintaining compassion and hope through every contact. We also keen to incorporate a trauma informed psychological approach which aims to increase people's independence, working towards an improved quality of life.

There is an emphasis on team based multi-disciplinary decision making which lends itself to creating a supportive environment for staff. We also value a culture of active service and personal development for all staff.

To support the triage and assessments of individuals who referred into the service alongside the wider multi-disciplinary team that includes community mental health nurses, psychology, consultant psychiatrist, occupational therapists, peer support (lived experience) workers, health care support workers and housing and financial advisors.

You will be responsible for undertaking assessments individually, alongside the wider team and/or alongside our partners in order to establish the person's goals, needs, risks and strengths to generate a formulation based plan of care with the individual and their support network

You will alongside wider colleagues in the team acting as a point of contact to provide clinical advice and guidance to patients and families where needed, as well as partner services for those being referred to the service

There will be opportunity where felt appropriate to provide brief interventions to individuals referred into the service to support their recovery and where it may prevent a need for more long term involvement from the service.

As part of the local mental health alliance network you will have opportunity to attend our weekly meetings to provide opportunity to look at how best as services we can work together to support individuals who are presenting with multiple needs, risks and complexity. Particularly where engagement may be difficult.
