Overview

The CCAN will work autonomously, managing a caseload of patients across various Mid and South Essex clinic sites, in collaboration with the Community Collaborative and as part of the Multi-disciplinary team, delivering individualised and personalised direct patient care.


Responsibilities

* Assess care needs and develop, implement and evaluate programmes of care without supervision.
* Teach other registered and non-registered nursing staff, educational professionals, parents/families and carers.
* In partnership with the multidisciplinary nursing team, develop, implement and evaluate a seamless specialist service for children and young people with asthma, ensuring that service users receive the highest standard of clinical care through evidence-based nursing care.


Financial and Employment Terms

As a newly appointed employee, you are responsible for incurring the cost of your initial DBS check relevant to your post; the amount will be deducted from your first salary with the Trust.
