Carer’s Link Officer Location: Oldham Council Contract: 6-month agency placement We are seeking a dedicated and compassionate Carer’s Link Officer to join the Carers’ Team at Oldham Council for a 6-month placement. This role will focus solely on the backlog of Carer’s Assessment Reviews, enabling the substantive team to concentrate on new and more complex cases. Role Overview Undertake Carer’s Assessments and Reviews, identifying needs and risks in line with assessed requirements. Develop care plans tailored to the carer’s needs, including access to information, advice, personal budgets, and carers’ services. Provide carers with clear information on services, welfare benefits, and charges to enable informed decisions. Liaise with statutory, voluntary, and independent organisations to promote the wellbeing of carers and strengthen local networks. Maintain accurate and up-to-date records in accordance with departmental policies. Support the delivery of carer-specific health and social care strategies across the borough. Ensure services reflect the needs of individuals from minority communities and socially disadvantaged groups. Key Requirements Health & Social Care qualification (minimum NVQ Level 3 or equivalent). Experience working in a social care environment (statutory, voluntary, or independent sector). Experience conducting carers’ assessments/reviews and producing written reports and care plans. Ability to identify needs and risks, and to make referrals or transfers to other appropriate services.
