The Opportunity
We are seeking a motivated and experienced Care Coordinator to join our growing domiciliary care team. This is a pivotal role where you will oversee the smooth coordination of care services, support our team of carers, and ensure that clients receive safe, timely, and high-quality care.
Based in our lively and supportive Acton office, you will work closely with colleagues, healthcare professionals, and stakeholders, while playing a key part in shaping the standard of care we deliver. This role offers genuine scope for professional growth, with opportunities to progress into senior management for the right candidate.
Key Responsibilities
· Coordinating with Local Authorities, healthcare professionals, GPs, and other providers.
· Conducting care needs assessments, risk assessments, service reviews, and quality checks.
· Supporting the rota team with weekly schedules and ensuring staff deployment runs smoothly.
· Maintaining accurate, up-to-date records on digital systems (Care Planner, Birdie).
· Ensuring compliance with contractual obligations and accurate data reporting.
· Preparing reports, attending meetings, and taking minutes for managers and stakeholders.
· Supporting audits and ensuring robust administration processes.
· Providing mentorship, guidance, and support to care staff.
· Handling ad hoc tasks and challenges with professionalism and efficiency.
What We're Looking For
· Previous experience as a Care Coordinator or in a similar Previous experience as a Care Coordinator or in a similar scheduling/administrative role within health or social care.
· Strong organisational and interpersonal skills with the ability to lead and motivate a team
· Knowledge of CQC regulations, care standards, and compliance requirements.
· Excellent communication skills to engage with carers, managers, and external partners.
· Confidence in report writing, record keeping, and administrative tasks.
· Ability to work well under pressure while maintaining attention to detail.
· Proficiency in IT and care management systems.
· A collaborative, problem-solving approach with a commitment to continuous improvement.
· Ability to work well under pressure and remain solution-focused
· Must have minimum 3-5 years in a similar role
· Level 5 or Degree in Health and Social Care
