Type of vacancy: Relief. Work pattern: Relief. Hours per week: As and when required.
Responsibilities
As a Relief Hub Facilitator you will provide high‑quality physical and emotional support to individuals, respecting privacy, dignity and specific needs. You will deliver day opportunities activities within departmental policies, legislation and practice standards, promote independence, and ensure risk management.
You will develop care plans in consultation with people, carers and other agencies; manage medication administration and safety; respond to crisis or emergency situations; and embed escalation procedures. You will oversee Hub Enablement workers and contribute to reviews, meetings and other stakeholder forums.
Qualifications
* A Social Care qualification or equivalent, with a care qualification or working towards Level 3 Diploma in Adult Care (including a supervisory role).
* Knowledge of relevant legislation.
* Evidence of continuous mandatory training and development.
* Knowledge of main ICT systems in the service area.
* Excellent professionalism, efficiency, and effectiveness in service delivery.
* Ability to collaborate well with colleagues and work independently.
* Effective communication skills with individuals at all levels.
* Experience of working with people with disabilities or in care settings.
* Experience of empowering and advocating on behalf of individuals.
* Experience of handling money and understanding financial guidelines.
* Experience supervising staff and delivering a wide range of day‑to‑day opportunities.
* Minimum of two years’ experience in service delivery in a care setting.
* Experience in planning, organising and delivering complex activities.
