The Stroke Coordinator team are looking for an experienced and motivated Band 4 who is seeking a new challenge within a well established multidisciplinary team supporting adults who have had a stroke. The post holder will support stroke survivors and their families to adjust to life after stroke, access community support and voluntary services to improve quality of life and promote independence. They will have strong links within the stroke pathway, including the Stroke Early Supported Discharge team, the Community Neurology Service and the Stroke Specialist Nurses at GHFT.

The successful post holder will work closely with the stroke coordinators delivering community review visits, telephone reviews and data collection and inputting into the National Stroke Audit.

The post holder will work independently as an autonomous practitioner reporting back to the Clinical Nurses and AHPs for the Stroke Coordinator Team. The post holder will be responsible for assisting qualified staff to facilitate timely reviews of patients within the community and aid the Stroke Coordinator team. To liaise with community and voluntary services to co-ordinate support for patients within the community. This will consist of referring and facilitating patients to self-refer to support services.

The post holder will be responsible for the coordination and implementation of 6-month Sentinel Stroke National Audit Program (SSNAP) reviews, and determining actions based on review; referring more complex needs back to Stroke Co-ordinators.

The post holder will be responsible for developing excellent working knowledge of the support available from partner organisations in health, social care and the voluntary sector to encourage and guide this group to access these services where appropriate.


Responsibilities

* Manage a caseload of patients, completing routine reviews and implementing support in line with plans set out by the Stroke Co-ordinator(s).
* Complete onward referrals to services within the NHS and voluntary sector.
* Contribute to inputting data into the SSNAP national audit system, with support from Stroke Co-ordinators and SSNAP inputters in other areas of the pathway locally and nationally.


Qualifications

The role requires appropriate qualification, training and experience as outlined in the Job Description/Person Specification. This role includes opportunities such as direct supervision, reflective practice/training, and meeting Care Certificate standards.

We provide a Multi-professional preceptorship programme aligned to the National Preceptorship Framework for Nursing. The programme is available to Newly Qualified Nurses, Nursing Associates, Internationally Educated Practitioners, Allied Health Professionals and Return to Practice Practitioners. There is also an option to pursue Level 7 (Masters) module Preceptorship for Healthcare (AP7026). Newly qualified Social Workers are offered ASYE.
