As part of the integrated service provision the post-holder will work within the Urgent Community Response (UCR) service to assist in the prevention of inappropriate hospital admissions and the provision of home‑based treatment in line with the overall objectives of the service. The post-holder will carry out personal care and assist the senior healthcare professional with acute care needs.

To support the UCR team in the delivery of safe, effective care to patients in their own homes and community settings, the post-holder will work alongside the team to provide practical support and contribute to traditional working safety, ensuring visits are carried out in line with Trust policies and risk assessments.


Responsibilities

* Carry out care and interventions under the direction of a registered professional, including manual handling of patients.
* Work in the capacity of a chaplain in line with competencies.
* Support UCR team during home visits for safety or risk management.
* Assist in maintaining personal safety by accompanying clinicians during visits to reduce lone working and following risk assessment procedures.
* Prepare equipment and environments for patient care, ensuring cleanliness and infection prevention standards are met.
* Observe and report any changes in a patient’s condition to the UCR clinician promptly.
* Support UCR team during visits where there are known risks (e.g., challenging behaviour, environmental risks, safeguarding concerns).
* Report incidents, near misses, or safety concerns promptly and accurately.
* Ensure timely inputting of relevant activity data within the designated timeframe agreed.
* Act to safeguard the patient, taking emergency action where necessary, reporting any potential or actual risks to a senior team member immediately and following the safeguarding policy.
* Respect the dignity, wishes and beliefs of patients and carers; involve them in shared decision making and obtain patient consent prior to all interventions. Patients may have barriers to understanding.
* Maintain clear, accurate and concise records according to Birmingham Community Healthcare Trust guidelines for clinical record keeping.
* Be aware of and able to deal with untoward incidences such as physical or verbal aggression, and adhere to Trust guidelines, policies and procedures.
* Take other duties commensurate with this grade of post in agreement with the relevant line manager.
* Work within the limits of competence and seek guidance when required.
* Communicate effectively both verbally and in writing with colleagues and stakeholders, utilising electronic methods of communication within the policy and procedures set out by information governance.
* Attend mandatory training, including lone working, conflict resolution, safeguarding, and infection prevention.
