Responsibilities
* Conduct assessments of clients' needs and circumstances.
* Develop and implement personalized care plans in collaboration with clients and stakeholders.
* Monitor and review client progress and adjust care plans as necessary.
* Provide advocacy and support to clients navigating services and resources.
* Maintain accurate and up-to-date client records in accordance with legal and organizational standards.
* Collaborate with multidisciplinary teams to coordinate care and services.
* Stay informed about local resources and services available to clients.
* Facilitate referrals to external services and community resources to support client needs.
Requirements
* A recognized qualification in social work or a related field.
* Current registration with the relevant regulatory body.
* Previous experience in social care or a related environment, particularly with adults.
* Strong communication and interpersonal skills to engage with clients and their families.
* Ability to assess and analyze complex situations and develop appropriate interventions.
* Knowledge of relevant legislative frameworks and policies related to adult care.
* Strong organizational skills and attention to detail in record-keeping and documentation.
