Job overview

The Perseid School in Morden is looking for Band 5 Registered Nurses with Special needs experience to join the Team on Bank as part of Epsom & St Helier University NHS Trust.

Please note, applicants will need to be able to commit to a long term booking, Monday - Friday 08:15-16:15

Key Requirements are Medication administration, enteral feeding, special needs experience/ school setting experience.

The School Nursing Team deliver health care to children with complex health needs aged 3-19 attending Perseid school. They are responsible for undertaking health assessments, completing and updating care plans, managing emergency situations and providing training for education staff. The post holder will ensure that high quality nursing care is delivered within Perseid school across three campuses.

The post holder will work collaboratively with other professionals involved in the care of children including Children’s Therapists, Educational Staff, School Nursing and Social Services to support and empower children, young people and their families, improve health and give every child the opportunity to develop their full potential.

Main duties of the job

1. The post-holder will ensure that all areas of practice adhere to all current NMC guidance.
2. Ensure high standards of nursing care are delivered at all times by supporting regular monitoring and evaluating patient outcomes.
3. Safely administer prescribed medications, monitor their effects and report all medication errors.
4. Ensure due regard is given to the customs, values and spiritual beliefs of patients and colleagues and that equal opportunities are practiced.
5. Takes a pro-active role in maintaining effective communication with all disciplines, children and families.
6. Maintain accurate and contemporaneous manual and electronic records in all aspects of care provision to promote continuity and partnership, in line with Sutton Health and Care policies and NMC Record Keeping Guidelines
7. Ensures that all complaints are listened to and that issues identified are addressed appropriately.
8. Use reflective practice as a method of ensuring that appropriate effective nursing care is delivered.
9. Reports incidents and near misses promptly and appropriately.
Promote a professional image at all times and be accountable for own clinical practice.
10. Ensures that nursing care is child focused and that children’s privacy and dignity are maintained at all times.

Detailed job description and main responsibilities

Clinical Skills

Work in partnership with the multidisciplinary team to ensure the health needs of the children and young people are met.

Carry out clinical care interventions daily for children with healthcare needs in the special school/ nursery setting.

To participate in teaching nursing and support staff as delegated by the Senior Staff Nurses/Clinical Service Lead.

To promote the mental, physical and emotional health of children with physical and learning disabilities liaising with parents/carers, teachers and the multi-disciplinary team.

To contribute to and undertake health assessments, to identify the health needs of individual children, implementing and evaluating programmes of care, referring as necessary to other professionals.

Work with parents and specialist teams to assist in the creation and delivery of care plans and risk assessments for pupils with complex health needs and long term conditions.

To follow the Sutton Health and Care Services and PAN London Safeguarding Children’s Board procedures to safeguard children. Provide reports and attend Safeguarding meetings as required

Identify risks when delivering care within the service, reporting these to the Senior Staff Nurses/Clinical Service Lead.

Contribute to the delivery and evaluation of the health promotion programmes within the school

To support senior colleagues in planning and providing appropriate learning experiences for students. Act as a Supervisor and Mentor for student nurses on placement in the school.

To utilise research findings and evidence based practice to inform clinical decision

Participate in any Clinical Audit programmes and the evaluation of own practice within the clinical area.
